I rarely evolve it first nowadays, but I often pick it over W at level 11. I guess if the enemy is very squishy you can opt into Duskblade and then you simply don't need Q, because you get so much cdr and often times one spell rotation is enough to kill. tl;dr depends on mythic - I evo R first with duskblade. 2.

What needs to be understood is manamune is not a good burst/assassin item its much better as a sustained dmg/poke item. That is why its a better item on bruiser kha than assassin. Now while I do think manamune is a good item on bruiser kha the problem lies in you building and stacking manamune before the game is over.
